The annual return is submitted with the accounts (audited if necessary)
and is due IIRC 10 months after the Accounting Period end. The AP is
normally the end of a month, so presumably this is a first set of
accounts for a company incorporated 12/09/2004?

No, if a company is a charity it sends annual returns to both bodies.
